Can confirm in 1.21.2.
Steps to reproduce:
Build an 8 block high pillar in creative mode.
Now place a block to the edge of it, on which you will need to stand and sneak to the pillar as close as you can..
Switch to survival mode.
Look up and place an anvil. ❌ Notice that you didn't took damage from it.
Now stand right next to the pillar and place an anvil. ✔ You will take damage from it
