When I access a chest, trade with a villager, or craft, my inventory duplicates items into the useage slots. This happens on survival or creative mode. It has caused my character to die twice.

It doesn't duplicate the inventory in my experience. It screws up the hotbar items, particularly those in the right half. This happens almost every time I access my inventory (be it through a chest, furnace, villager, ...) and is super disruptive to game play. (Particularly when on a horse and I can't change the hotbar items without dismounting ). I have also had inventory items completely disappear when trading with a villager, which may be related.

These are called Ghost items. Unfortunately I do not know a solution to fixing it other than just ignoring it, but there will be an upcoming update to MCPE that fixes this
