I have been losing items when dying. I tested in an open field to make sure I wasn't just missing some items. Upon death, in hard home without keep inventory on when I die some items do not drop. I've lost mostly armor, chest plates and leggings. Sometimes random other things. I'm not sure what is causing it, but curse of vanishing is not enchanted on anything.
