I was able to make this work by correcting your command.
In this command you are trying to detect a "command_block 0" ("diamond_block 0" in the screenshot). The data value 0 for a command block means that it is pointing downward, but in the screenshot your command block is pointing to the side. If you change the data value to match the orientation of the command block it will work. You could also switch to using the diamond block, whose data value is always 0.
