On my realm I have had three occasions in which a player has entered a nether portal and instead of being transported to the corresponding coordinates in the nether (if coming from the overworld) or in the overworld (if coming from the nether) they have instead found themselves in the exact same coordinates in the corresponding dimension. In other words it is as if the game forgets to divide by 8 or multiply by 8 for the position they are teleporting too .So far one player has had it happen twice in one portal coming from the overworld into the nether and another player has reported it happening to him once in a different portal coming from the nether into the overworld.

Not sure if this is related, but on our realm BOTH times when this happened to a player, they were eating while beginning the transportation cycle of the portal. Is this coincidence or has anyone else tested whether this is a verified part of the bug?
Eating does seem to cause this. A youtuber named silentwisperer made a video on this, Google "silentwisperer broken nether portals" and it should come up.
This just happened to me on a realm, and I'm using Windows 10, Minecraft version 1.10.1. It transported me to the nether at the same coordinates the overworld portal was that I entered. I also was eating. I'm going to try and recreate this when I get back to the portal in the overworld.
Edit: I was able to duplicate it and it seemed that i had to finish eating at the moment the portal transported me. The first time it happened, there was no nether portal generated in the nether when I arrived. I placed a quartz block with a torch on it and tunneled to the nearest portal on the realm. The second time it happened, a portal did generate! Of course it doesn't take you back to where you came from, so I destroyed it. The third time, a portal did not generate. This is the one I captured on video. Each time I entered the nether at the same place.

Apparently after 1.16.210.05 it started happening again. Many players on my Custom Bedrock Server complain about being teleported to random locations when going through portal. They don’t need to eat to trigger this bug, it doesn’t happen every time but randomly. The distances are sometimes up to 100k blocks away from correct destination.
I found something where it makes a new portal in the overworld when you go back through. This is because if you make a portal and the one in the nether has blocks where it would spawn, it spawns it somewhere else. That area isn't blocked off when you go back to the overworld, making a new portal sometimes hundreds of blocks away. I fixed this by clearing out the area in the nether where the portal would've spawned, then broke the portal in the nether and made a new one in the space I cleared out. It linked to the portal in the overworld and fixed it.
The player who encountered the bug going from nether to overworld was using Windows 10 Edition at the time.
