With a tamed parrot on the player's shoulder, if the player jumps forward off of a one or two block height, and the player's head hits a wall in front of the player while falling, the parrot disappears and suffocates in the wall.

I was not able to reproduce this problem in Windows 10 using 1.11.4. Each time I jumped against a wall with a parrot on my shoulder, either the parrot was knocked off and flew to the ground, or remained on my should and did not suffocate.
It might be that I don't have the blocks configured right. Could you provide a picture or explanation of how the blocks are set up? Or a copy of the world with your player in position, ready to jump?
Another possibility is that your player's model is defined incorrectly, causing the parrot to ride your shoulder in a slightly different position than it should. Are you using a custom skin or one from a resource pack? If the latter, what are the pack and skin names, and if you didn't get it from the Marketplace, is it available as a free download? I'd like to try using the same skin.
