There are holes generated in the nether which go from the ceiling to the bottom. I have ran into these while digging tunnels, and randomly exploring the nether. I was playing on a realm if this matters at all. (no it wasn't a player doing it)

Having almost died to some magma cubes and pigmen, I was making my way back to my nether portal when I saw some pigmen through a 1 block tall gap into a cave I hadn't discovered yet.
I started to mine toward the cave when I found myself plummeting through a 1 by 1 hole down through the nether into lava.
I lost many things, including my loyalty trident, enchanted diamond sword, other tools, and of course the 27 levels I had gathered through mining stacks of quartz.
To summarise: It's not a good thing having tiny but deadly 1x1 holes through the nether that have no relation or connection to the terrain around them.
I can say by experience that losing everything on you not even to an evil nether beast, but to a suspiciously tall and thin hole is very dissatisfying.
It's not a common bug, it doesn't effect everyone, but it can be deadly unfair when you come across it.
If you take anything from this, it is to always have a fire resistance potion on you. Maybe even two. And also feather falling in case there's no lava. 😛
If this is a feature it should be removed because it is so random and really takes some of the fun out of the nether. If you die early on with some good stuff it is near impossible to get your stuff back. I have only seen them in some worlds and in some like 5 per chunk and others 2 per chunk it needs to be fixed very bad I almost died because of them. Lucky I had a raid farm.
