This issue does happen on Xbox, android and Nintendo switch for me aswell. I'm not sure what causes this issue. I listed it as a windows 10 bug as that was the version I happened to be on at the time.
I've created a Redstone 2x2 door in my world and for some odd reason one of the pistons usually has a piece of stone from my door moved backward (with no pistons or sticky pistons to push/ pull it backward.) This then results in my door being not so hidden like its meant to be and therefor open and obvious to anyone that walks past that there is a "hidden" door there.
Below are 2 images. 1 is when the door is open and 2 is when the door is closed and everything is in the correct places.
