Multiple Bad Omen Levels are a Java Edition exclusive feature. This means that it is not possible to occur on bedrock edition. On bedrock, there is only one Bad Omen level which yields the same effect. If you are on Java edition, post this issue on the Java edition Jira. I hope this helps you.
