Once you enter a nether portal,theres a chance you can take damage through suffocation.
This usually occurs when nether portals are spawned in tight locations and once you enter you suffocate for a secind and take damage,although i dont lose hearts on my experience...
AND I SWEAR TO GOD IF SOMEONE ALREADY REPORTED THIS,I SWEAR LOOKED IT UP AND FOUND NOTHING ABOUT THIS SUFFOCATION GLITCH.
P.S.-Its not because of the blocks... Its just when you enter ONE portal BLOCK and enter a tight area.

Kinda not the portals fault,its the surroundings,it might just be because i move alot when i enter a nether portal. But sometimes if i stand still it still happens,probably because i entered a summoned 1x1 portal block,entered it and thought that the nether portal on the other side (the nether) should also be 1x1 but it isn't.

have you tried making a huge portal and if you still suffocate? Because they made it so you can make portal really big. That might give a difference...
