Resetting the nether using a third party tool is the cause of the issue. Deleting the nether does not remove the portal links because you've directly modified the game data. In order to get new portals created in the nether you have to delete the nether-side (DimId: 1) portal entries.
Use of third party tools for world editing is not supported or recommended.
I updated my world and deleted my Nether and uploaded the map to Realms. When going from the Nether to the Overworld the portals link up. When going from Overworld to Nether the portals aren't linked and it teleports me and doesn't make a new portal. It also teleported me into a lava lake.

This happened in a copy of my realm that I was in the process of converting, luckily it was a creative copy and I found a safe portal to use to get in and rebuild my netherside portal
This same issue is happening for me on 1.16 Java Edition. I have a multiplayer server that is hosted on ggservers so it is a "third party" but I do not now how I would reset the links otherwise.
I found a fix not a easy fix as I just spent all the time running through portals. I used the same program to wipe the portals. If I was a smart person I would have been able to figure out which portals were which. I went through the portals from the Nether side broke the new portals and they finally linked up. Some put me in weird places, like under my ones friends base in a spider dungeon.
Making a portal in the over-world and then going to the Nether, Leaving you stuck with out a portal to get back was happening in 1.14. Deleting the portal records list does fix the issue, if you had deleted the Nether using Mcctoolchest. or World Painter.
I rebuilt the portal I am having trouble with. Now I am being dropped. The portals I dont have an issue were my old ones before the reset. I even rebuilt both sides.
As the note says, using third-party tools is not recommended - in fact I think this bug should be resolved as invalid, but let the staff decide.
However if you wish to delete portal data use the following steps - to my memory I had to delete all portal data for it to resolve. This will not delete the portal blocks themselves.

Please note that after deleting the Nether, the Nether Fortress data from the previous Nether will still be carried over, i.e. Wither Skeletons, Blazes will spawn in the same location where a Fortress once was. This is also probably not supported by the bug tracker.
If you are experiencing portals not linking up correctly even though you have never used a 3rd-party tool, you are likely not affected by this bug and in fact you are doing it incorrectly - when you go from the Overworld to the Nether, the game will search for another portal within 128 blocks of the pre-calculated spawning position. It is also 128 blocks the other way around, so if you have a portal at 0, 0 Nether, a portal at 450, 300 Overworld will link to it because divided by 8 is around 56, 38 Nether, less than 128 blocks.
Alternatively the game will try to spawn a portal at the nearest valid location, so a portal at 0,0 Overworld can spawn one at 80, 60 Nether if everything from -79, -59 to 79, 59 is blocked (unable to spawn portal "safely"), which is why when you go back you will end up at a new 640, 480 Overworld portal.

tldr, resolve this "bug" by deleting all portal data, if your portals still arent linking correctly and/or you have never used 3rd party tools you are probably doing it wrongly
Also note that Mojang does not offer a Nether "reset" alternative, but if you want Mojang to add one there is a feedback post for it

The server you are playing on is modified.
If you can reproduce the issue in a vanilla environment, please recreate the issue.
Any non-standard client/server build needs to be taken up with the appropriate team, not Mojang.
Any plugin issues need to be addressed to the creator of the plugin, add-on or resource pack.
If you have problems on large servers, such as Mineplex and Lifeboat, please contact them first as they run modified server software.

I get that using a Third party tool was going to be an issue. However, last time the Nether updated I was able to reset the Nether. I was optimistic about the Nether having a reset again. When I found out it didn't, I took matters into my own hands.
