The maps are wonky. Not the world itself, but the paper maps.
When going out of a map's borders, or beyond the edge of the map, the icons that represent players get teleported into the middle of the map. It just incorrectly states the player's position, basically.
To recreate this bug, simply hold a map (the ones I had were level 3 and 4, so I'm not sure if it affects level 0-2, but it probably does) and walk outside of the borders. The player icon will be teleported from the direction they were going in into the middle of the map. The player itself will not be affected, thankfully. Walking back inside the map's range will once again display the player icon correctly, but will bug out once the player steps outside the boundary again.
The image below is of the map in my hand while it incorrectly displays my position. My character icon says I'm in the middle of the ocean, but my surroundings say otherwise. My actual position would be to the north.
