It worked fine before the aquatic update. Now, when me and my friend are both in the realm, my second friend cannot join. It seems to think it's full and gives the message "Wow this server is popular! Check back later to see if space opens up."
It's not until I leave the game, my 2nd friend joins, and I can join back.

This bug is NOT resolved.
Having this same issue on Xbox One, and was directed to this bug by Microsoft Support.
If I sign in on my account (realm owner), only one other player can join. A third player (second "friend") cannot join, and is given the "Server Full" message.
However, if I sign out, two other players can join the realm, and I am able to join as the third. A fourth player gets the "Server Full" message.
All four of us were able to play on the same Xbox One with the "2 friend" Realm prior to this.
I, too, have encountered this issue - and it is still outstanding.
Stumbled onto this issue after Microsoft Support couldn't assist.
Still happening on the owner plus two tier - I play with my two kids, and one will get kicked off on a regular cycle (tics?) and can't reconnect because it'll say that the server is "2/2" full. (Wow this server is really popular! message). If I (the owner) disconnect, then they can reconnect and then I can reconnect. Is the issue that the owner is counting towards the 2/2 playercount? If so, then it should change to a 3 total. Otherwise, isn't there a way to flag the owner so they won't count in that realm playercount.
Any updates on this issue? I am afraid I can not reproduce it.
@galiean251 - the 2/2 Realm should be able to host 2 guests and the owner so it is dedicated for 3 players.
Cleaning up old tickets: This ticket had been set to 'Awaiting Response', but has not received a response from the reporter (~3 months+) so is being closed as Cannot Reproduce. I've had the same issue. I've emailed support and have been told this is a known issue they're working on.
