Crash when Ghast Fireball entering a Nether Portal

Context: 1 Portal in the nether, 2 in the overworld
Portal Overworld.A -> Nether.A
Portal Overworld.B -> Nether.A
Portal Nether.A -> Overworld.B

Upon entering through Overworld.A Portal, I spawned at portal Nether.A. A few seconds after being in there, I was starting to go through again when a ghast fired a fireball, it passed through into the overworld.
Upon entering Nether.A Portal to go to Overworld.B Portal (Which leads to the stronghold/end portal frame [opened]), I crashed along with my friend in the realm. This also caused the realm to load a backup around 5-10m before.

Key Notes: The End frame was in a unloaded chunk, noone was there.
Friend was around 2000 blocks away at his base.

Assumed issue.
[Not Proven, This is a opinion.] The Ghast Fireball spawned inside the portal, but due to being unloaded, just stayed there until the chunk was loaded in. Once I entered, I spawned at the exact location of the ghast fireball and collided, this collision appeared before I fully loaded in and the portal broke on the overworld. This then caused the game to try to generate another portal and send me through it but then caused the game to create two of me? Unsure, not a programmer but this is what I assumed may be the case? [This is most likely not the case, as this is an assumption with my limited knowledge].

Longer Version
I entered a portal that was around 500 blocks away from the second portal in the overworld. These two portals lead to the same end in the nether, but the nether leads to only one of them, that being the one in the stronghold end portal. Upon exiting through the nether end, I walked a few blocks out and reentered the nether end to head to the stronghold nether portal to enter the end portal. Before I could enter the nether portal, a ghast shot through the portal and the fireball appeared to have entered through the other end. Upon entering through the portal, the realm disconnected me and a friend, but loaded a backup for us to rejoin. Noone was on the other end of the end nether portal (End nether portal referring to the nether portal inside a stronghold, next to the end frame).

Control Test:(Fail) (Appears to be unloaded chunk test)

It appears to not have been a ghast fireball but nothing at all. I'm unsure exactly why this is and could not recreate the bug in singleplayer after a few attempts.
