Player safety report form captcha causes error

upon green submit button causes a popup to enter characters for a captcha, filling that out correctly or incorrectly consistently returns with an error occurred. It's been doing this for quite some time. 

macOS, Chrome (mac mini m1)

iPadOS, Safari (iPad pro)

iOS, Opera (iPhone 11 pro max)

iOS, Safari (iPhone 15 pro max)

with or without uBlock origin or adblocker, can reproduce.
