Also encountered this today, specifically with a bow from a skeleton. Could apply to more mobs/items, might try to check if pigmen or wither skeltons can drop 0 durability swords.

I'm still having this problem in 16w06a.
I'm in a multiplayer LAN world and I take damage in portals (about 16 hits), while the host player doesn't.
