Had this happen to me in 1.16.4, vanilla multiplayer server. I had a shulker box with some diamond blocks and some diamonds. Shulker box was in the first position in the ender chest, diamonds and diamond blocks in the first 2 positions of the shulker box.
I am 100% sure this shulker box or the diamonds have not left my ender chest for a few days. I am also 100% sure the diamond blocks and diamonds were still there the last time I checked it. I then check the shulker box, the first two slots are empty.
