The same thing happened to me today. I threw my trident (loyalty 1, sharpness 3) on a creeper which was standing on leafs.
The trident dealt damages to him (he turned red and hearts appeared above him), but it never came back, and wasn't on the floor either.
I thought it went through the leafs, but it wasn't under.
