This happens to me about 1 out of every 4 attempts coming back to the over world on 1.13.1 windows 10. The workaround I've fond is to re light the nether side portal from the bottom.

I play on windows 10 and this bug just happened to me exiting the nether going back into the over world. Minecraft put me at the same coronets in the over world ie I was at -1000 64 200 in the nether and was dropped there in the over world

UPDATE: I was able to fix the issue by relighting the nether side of the effected portal so exercise caution and relight your portals as needed this has happened multiple time when leaving the nether the game doesn't multiply the coordinates for the over wold.

I just died going through a portal this issue isn’t fixed at all

Both phantoms and ghasts don't despawn for me either on single player and dedicated server

I have this issue on window 10 as well

sine the 1.13 update this has been happening to every farm I build that uses minecarts

This happens to me on dedicated servers and single player as well. it happens with different types of food including steak golden carrots and golden apples as well.

This is happening to me on single player worlds realms and servers. the nether is pretty much inaccessible.
