The same thing happened to me. First time on my server to place a portal I went through and was killed in lava... second time I went through the same portal and there was no place to fall/an unactivated portal was at a 90 degree angle to the one I walked through. Very strange...
I'm on seizure meds and this issue is making it very difficult to have a chicken farm without redstone. Please fix this.
