Still in 16w05b. I also found a way to turn those ghost items (That were supposed not exist anymore) into normal items again (Which I think is a big problem) In order to do so, try this: Do what is described in the original report, you should end up with the ghost stack of torches in your inventory (They need to have been in your inventory first), now simply grab the stack and drag them outside of the GUI so that you will drop them (The drop needs to happen this way, you can't press Q), and that's it, those items that you dropped are not ghost anymore. When you try to pick them up, since they are real items again the "/clear" command will clear them, so make sure you turn it off before picking them up.
Oh, ok, i didn't thought the second selector in the operation could be the problem. In that case, @a[c=1] will yield the expected result while doing the same thing as a @p right?
