This happens seemingly at random. in creative or survival worlds equally. but it does sometimes occasionally happen and the only way to fix it is to save and quit then go back on the map again.
It might be a possibility that it happens when playing a new map for the first time. or playing for an extended period of time. normally it happens around or near where you are playing but off in the distance where you can't see till you get close enough for your chunks to render into view distance
its not exactly replicable but it does in fact happen. Happens with, OR without mods
There are two different variations of this mysterious "hole" or chunk error. Some are real blocks and have lighting working properly. (You can pour water down and explore them).
And the other is Fake where as in you can't do anything to it unless in creative. and lighting does not work and shows as black inside. You can walk over this type of hole as if the blocks are there but very slowly. You can't fall into them and mobs can walk right over them with ease.
