So I was on what I believe was the latest driver version via windows updates for surface pro 3. After seeing your comment, I updated to intel driver 20.19.15.4300 (date is 10/1/2015). Crash is still reproducible. I will attach my latest dxDiag named DxDiag_11_16.txt

Updated java to latest runtime just in case. Crash still occurs.
