This has been happening to me recently on an Iron farm off the main area in the end. 20 villagers were carefully nametagged and placed in a 7x7 confined area. No problems at first, then it was brought down by half suddenly (10 villagers). Then I replaced those missing villagers with ones nametagged separately 1 through 10. The next time I checked, a random 4 of those sequentially named villagers were missing, with 2 left over from the initial 20.
