I have had this happen once or twice. When you created your portal in your base the corresponding portal in the neather was not spawned in the correct location.
Write down the X and Z coordinates of your portal that is in your base. (viewable by hitting F3 in game)
Divide X by 8 and Z by 8.
Those numbers are the proper X and Z coordinates of the portal in the neather. My guess is that the neather portal that was automatically spawned is about 10-12 blocks away from the proper neather portal coordinates.
Best thing to do is break the natural neather portal and rebuild it in the proper coordinates.

Correct me if I am wrong but this is what I have found on the new repair/enchantment system.
The first time I repaired a pickaxe it is very cheap (3-5 levels on a fortune II, Efficiency III, Unbreaking III pickaxe), the second time I repaired the axe it is more expensive (15-ish levels), and the 3rd time I tried to repair the pickaxe was going to cost something around 30 levels so I let it be destroyed.
If your sword has been repaired more than 3 times I bet that you have reached its limit for being repaired =-\

yes this happens but it also has done it in 1.7.10 , 1.7.9, 14w34c and 14w34b.
