I was playing around it seems to only happen when the player is facing North and places a piston on the top side of a block so that the piston head faces south. Interestingly while the top side has what looks like a double head on one end the bottom side has a head on both the North and South side. When the piston is powered it functions as it should and the ghost piston heads persist.
