I'm having the same issue, I traveled through the nether and came out a portal then walked a few hundred blocks and placed a portal near an end portal and when I go through it starts generating nether world and the game disconnects. Then when i try reconnecting it says can't connect to realm and I have to wait a few minutes until it resets the world back a few minutes before trying to enter the portal.
