Strangely enough, something similar had happened on the realm my partner and I share. We've had it happen now a few times:
1) I exited a nether portal, placed a bed, slept, woke up and broke the bed. I confirmed it was in my inventory, then logged out. Next day when I logged in, the bed was in my inventory but also respawned where I had it before outside the portal (duplicating itself)
2) Partner was wandering in desert between villages when he noticed a stretched/in-use lead sticking out of the sand but nothing attached. Walked closer and a boat with two villagers appeared on the other end - the exact boat and exact two villagers he transported along that route to the second village days ago (boat, lead, two villagers duplicated).
We are playing a Bedrock realm and each on separate PS4s. We have not tried playing locally to see if the issue persists.
This just happened to my husband and I playing our realm on separate PS4s (10/8).
I was chopping trees and crafting oak steps in a semi-remote area and he was chopping trees near one of our villages when we noticed the signs of disconnecting. Neither of us could open inventories, interact with placed items (chests, crafting tables, beds, etc), mobs were frozen in place. We exited the game, then upon trying to reload we didn't get passed the loading screens and either had to cancel the operation or we would get an "Unable to connect to world" message. We tried uploading an old save from 7 minutes, 37 minutes, and 8 hours before the disconnect and everything failed to upload. About an hour after the crash, we tried to log back in and it loaded the save from 7 minutes prior to the crash.
This happened more minorly 10/7, where we were able to upload an old save and only lose about 30 minutes of work.
Again on 10/9 while playing Realms with two of us on separate PS4s and one person on Xbox, the game froze. We all left the world and it again won't load back in (stuck at the "Loading resource packs" page). We'll try back in a while.
This happened to me too (Bedrock realms, PS4). I lost two villagers that had been stable in their trading stalls for months. Nowhere near chunk boundaries, and no possibility of hostile mobs nearby.
