I just realized that it could be both off hand and main hand packets....

It's not an issue in 15w44b. It must have been fixed around the time they added "The Void" preset.

Confirmed for 15w44b! Another problem with the bossbar is that when you add the tag "CustomName:<name>" then "<name>" isn't over the bar (same for withers)
