This happens to me too. I can connect to the worlds of friends who are in other states but can’t connect to my friend who is sitting right next to me on the same couch.
I found a workaround by connecting one iOS device (iPad) to the WiFi and the other iOS (a different iPad) to my iPhone’s hotspot. Then I was able to connect to the same world with both iPads.
I’ll read the NAT instructions again but it wasn’t clear how to do this with an iOS device.
