I noticed this same problem on iron shovels with efficiency III, and diammond shovels with efficiency III (at least) possible with IV too. I am a programmer and have played minecraft a lot so, what I think its that somehow its so fast that the update of the chunk gets somehow wrong (as if it where done in paralell and one is overriten). as this does NOT happen when trying to do it slowly (barelly pressing the mouse). but truly not the idea. also, I Had that problem once digging down (from a 1 line of dirt), and i got stuck and sofocating, lucky i jumped a few times and was able to jump to a side (but if will have done it straight down mostly sure will have been fully sofocated). the version i tested this is in 1.12.2
