I kept trying to recreate the issue, and finally got it. It happened after nightfall (all the villagers ran inside). I went into my house, closed the door. I unloaded my entire inventory into a chest, and left a stack of dirt in my inventory. I saved and quit. I did not close out of the application, just clicked on my world to bring it back up. If you look at the world selection screen, you'll see the image for my world shows the inside of my house, looking at my chests. Once my world loaded up, that's when I got the 'you have died' screen. If you look at the background, it is outside. It is raining, and while I couldn't capture it in a screenshot, an enderman appeared to bounce into and out of frame. Another interesting thing to note, is that it shows cobblestone and mining picks on the ground. I didn't have either of those items in my inventory, only dirt. The biome looked similar to my current biome, so after I respawned, once again back at my house staring at the chest I had saved at, I ran outside, ran outside my village, and looked for the 'scene of the crime'. The picture shows a meadow and pond/lake. There isn't anything like that within a 100 blocks of where I saved & quit. I will also note, that while I couldn't find the location in the picture, it WAS raining in my biome. So there seems to be at least some connection?
