I've seen this twice on my Samsung Galaxy S2, a Survival mode world is converted to Creative. I'm running Android version 4.0.4.
I suspect it has something to do with the application crashing. I don't know how to replicate, as it doesn't seem to be associated with any particular activity, but since 0.6.1 I've had several crashes which required a full reboot of my phone. Seems to happen more often when hosting multiple players.
