Just Saying
I think u guys use Mob head drops/Player Head Drops Datapack of Vanilla tweaks or u guys have player head drops on on the server

I faced this same issue 2 months back came to this site and couldnt get any help

just somewhere i read hermitcraft faced the same issue
only thing common btw my and hermitcraft server was player head drops and mob head drops

i went back Started an event on my server saying i will pay 5 diamonds per head u bring me

they brought me like 200+ head that day
i went to creative cleared my inv

after 24 hrs of my server on 
the bad chunks
were absolutely Fine

it worked for me try for urself
