Can you provide the last game log?
Usually located at: AppData\Roaming\.minecraft\logsMost recent log file is named: latest.log
If you cannot find the above file please send the one with the most recent date.
Mojang won't fix this (although it's probably your computer) because you are using a modded client aka OptiFine.
