Given how old that report was and that it was marked as resolved, I opted to create a new report rather than add to that one. This submission also better describes the issue and provides an up to date example in a normally generated world.
I can contribute two examples of this bug in 1.16 (last tested 1.16.3):

Unfortunately, this bug has been rare enough in past versions that it is not feasible to reproduce without a seed finder. Until a seed finding tool/program has UTD cave gen code, there is very little chance of finding an UTD example.
This would also be true for MC-163945, though to a lesser extent as that bug is relatively more common.
