Hi, search for "ghost blocks". It is a common bug that has to do with mismatch between what your client thinks there is versus what the server thinks there is. Number 2 also seems like it is related to client-server communication. Try not pushing yourself into the wall when mining. This could be enough to solve this problem.

I'm having the same problem. I'm going to guess that the gold ore intentionally doesn't spawn as they are probably going to replace it with the guilded blackstone in that biome. But the lack of ancient debris seems like a bug. Probably the basalt and blackstone spawn after it and override it.

I'm experiencing the same issue. I didn't use commands to replace blocks, I just used beds to blow everything up below y = 20 approximately. I didn't find any ancient debris in a few dozen chunks in that biome but as soon as I got to a different biome I was getting veins almost once per chunk. So it seems like the blackstone and basalt veins override the ancient debris (unless it intentionally doesn't spawn in that biome?).
