This issue has been fixed for me. It turns out it was a faulty ram module that was causing it.

Having the same issue with and without mods installed. I started happening sometime after installing new ram but around the same time I also updated my graphics driver. I did a memory test on the ram and no issues where found plus only Minecraft has issues not any other game.
