Something similar has happened to me in the past. It turned out that the server would occasionally lag, causing the piston to not have a 0-tick pulse and thus breaking the cactus/bamboo.

That is probably the reason why the stuff is disappearing randomly.

Also occurs in 1.14.2

The guardians never fall thru the lava killing chamber of the farm, but instead most of them end up dying in the lava (only 1 block tall).
