Also had this happen in 1.19.81 was in the nether on a realm (unsure if that's relevant) threw a Loyalty III trident at the ghast and the trident just never came back. There was no reason to believe it had fallen into lava to cause the disappearance and the durability was such even if it had taken damage from the lava it should have still returned.
This issue also does not seem limited to the Nether. I recently threw another Loyalty III trident in a Warm Ocean biome only to have it never return either. No reason to believe it had broken because durability was high. Tried exploring the area where it was thrown and could not find it.
