it only has happened once while playing on bedrocks "lan" feature so basically multiplayer
i have had this issue too but mine was a world save corruption issue. it corrupts upon saving or loading and it is quite annoying so backup your save often as a temp fix
and its windows 10 edition. its in the environment list
