I have the Prizma Visuals addon but it wasn’t used on this world. The world was created around the 14th of July.
Was playing in Preview, multiplayer enabled but not used.
Also to @Hyde in case you haven’t tried already, try find water to drown in, lava to burn in, gravel to suffocate in or wait for a hostile mob to spawn and kill you. When you respawn you should return to the surface world spawn.
