There are multiple actions, other than switching weapons or attacking, which cause the player's attack cooldown to activate. These actions do not cause the attack indicator to appear, nor does the weapon lower to show that the weapon is in cooldown.
These actions include:
1. Placing a block from the offhand
2. Tossing a fishing rod from the offhand
3. Clicking a block in Adventure mode you can't break
Upon performing any of these actions, the cooldown is activated, but the game does not communicate this to the player.
To replicate easily, hold an axe in the main hand, and a fishing rod in the offhand. Then, use the rod and immediately afterward attack a mob. It will do significantly reduced damage, indicating that the attack cooldown is in place but not being visually displayed to the player.

You seem to have misread my post, so I'll edit it. The scenarios I described are indeed not affected by the cooldown, but they affect the attack cooldown. Placing/breaking blocks, using a fishing rod from offhand, and attempting to break a block in Adventure Mode all cause the weapon in the main hand to experience attack cooldown, but the game does not indicate that this cooldown is occurring.

You're right, tryashtar; together those reports cover the whole issue of the attack cooldown indicator.
I don't imagine any of these will get fixed because of Combat Update v2.
Works as intended. The scenarios you described are not affected by the cooldown. Placing, removing blocks and all right click functions are all instant. It's called an attack cooldown, not an item cooldown.
