Three chests in a set of underwater ruins spawn with the exact same items (number, position, type, enchantment) – thought this was loot table based so it shouldn't be spawning like this.

I'm experiencing the same thing.
Six chests in a nether fortress, each with the same 3 items (diamond horse armour, gold horse armour and 1 gold ingot) in the same arrangement. Wish I'd thought to take screenshots, but I thought it was just a fluke at first.
