Attribute modifiers randomly run out when given via items

When wearing armor with attributes on them, sometimes they'll just run out, even when you still have the armor on.

You can force this to happen by switching to or from an item that modifies the same attribute in your hand.

But it's still a bug though...

Yes, I was just giving an easy repro.

The same happen to Mobs, when you give them items with custom attributes on it. Best for testing is to modify the speed attribute modfier on an item(for example +0.2145). If you pass this to an zombie, his speed will increase, but randomly jump pack to the default speed of the zombie and stay there. If you leave the world and load it again, the zombie will increase his speed again.

May be related to MC-16279. I've noticed in MC-16279 that mobs are guaranteed to lose their Attribute Modifiers if you wander a certain distance from them. As to what condition causes players to lose attributes, I have no clue.

Also seems "Pause/Game menu" causes it to stop working too.

@unknown, between the changes to the attribute system, and the additional cause, that's likely a new issue, although similar to this one. Please file a new issue, and provide steps to reproduce.
