Health boost resets the additional health every 30 seconds.
Steps to reproduce:
/effect <player> 21 300 2
Wait for health to regenerate naturally
At every 30 seconds the additional hearts are empty again

You can use the SethBling's MCEdit filter to add hearts on an armor

His last filter, "AddAttribute", apply it on a chest where your armor/item is.
