Alternatively, this could be MC-211181 instead of MC-192290. The reporter specifically mentions 21w07a in the description, which implies that this might not have happened for them in 1.16.5. (MC-211181 was introduced in the caves & cliffs snapshots and does not occur in 1.16.5; in contrast, MC-192290 has affected versions from the nether update.)
@unknown, does this occur for you in 1.16.5? Also, what graphics card and operating system do you use?
