The world icons will be wrong frequently.
Here are three occasions that was discovered by me so far:
A world icon was abnormally displayed in front of another world.
After saving a world, the icon could not display. But when backed to the menu screen and click "Singleplayer", the world icon displayed!
A world icon can not be saved.
Among them, only the 2nd and 3rd occasion can I attach videos below. The 1st occasion is a probobility event (and it must be reproduced via opening a lot of worlds).

Please attach the game log from when this happened. You can find it in %appdata%/.minecraft/logs.
Perhaps there are some error messages there that might be of note.
I added the game log and this case appeared when I saved a world and returned to the worlds selecting interface.

However, this issue is Invalid.
Your game, launcher or server is modified.
If you can reproduce the issue in a vanilla environment, please recreate the issue.
Any non-standard client/server/launcher build needs to be taken up with the appropriate team, not Mojang.
Any plugin issues need to be addressed to the creator of the plugin or resource pack.
If you have problems on large servers, such as The Hive and Hypixel, please contact them first as they run modified server software.

1: sounds like MC-181280.
3: At least a certain number of chunks need to be visible in order for a world icon to be generated. Chunks with only air don't count.
