Command block mode not working after NBT-pick block

There is a longstanding issue with using ctrl+middle click on command blocks that are set to "Repeat, Always Active". (This may also apply to "Requires Redstone", I only had it happen once a long time ago)

Rarely, the block you receive is bugged and doesn't begin executing when placed down. In order to get it to start functioning, you must cycle through the modes (Impulse, Chain, Repeat)

Steps to reproduce (This happens very rarely with no obvious reason)
1: Place a command block and enter a command
2: Set the command block to "Repeat, Always Active"
3: Use ctrl+middle click to receive the command block in your inventory with the current NBT data
4: Place the new block back down (which is in the "Repeat, Always Active" state) and observe that it does not function
^ N.B.: This does not always happen and I have not seen any correlation between the commands or environments during each instance of this bug.

You can place down a bunch and they will all be broken, the error is tied to the item in your inventory.

Running "/data get block" on it shows the same NBT as the source block, there are no obvious errors.

The world from this video is attached, and the bugged command block is in the player's inventory, ready to be placed and/or inspected. The world is in a 2-part zip file due to the Mojira attachment size limits, and both files must be downloaded and placed in the same folder before the .zip file is opened.

This issue is particularly frustrating when you're not sure if your code is correct, as I often spend 20-30 minutes re-writing things multiple times before realizing I've run into this bug again.

The attached zip file cannot be opened/is corrupt or empty. Please attach a proper zipped world.

Also, please attach steps to reproduce getting the bugged command block in the first place. That'll greatly help understanding this issue.

I cannot reproduce this reliably, this is the first time I've recorded the resulting broken block. This error occurs seemingly randomly, and very rarely. Maybe 1/100 with my usage patterns.

I too am unable to extract the zip contents. If you're still able to reproduce this in 1.20.4 or later, please reattach the zip files, and if possible correct the YouTube link as the video can't be viewed.
