for some reason when i have an item in my offhand i cant attack with a sword further more i cannot attack once i take that item off of my offhand everytime i try to attack it triggers the offhand item instead for instance when i have a sword and shield i try to attack with the sword by clicking but it blocks with the shield

Try assigning attack to a different key (like I don't know, M) and see if pressing that key still makes this issue happen.
I can't seem to replicate this either. Would be great if you could demonstrate it occurring in a video.
Please add a video showing this issue and showing the change in keybinds to ensure that doesn't play a part.
If you are on Windows, you can use Windows+Alt+R to open a built-in app for recording game footage.
If you are on Mac (Mojave or later), you can use Shift+Command+5 to open a built-in app for recording your screen.
In case you don't have a program to record videos, we recommend using the free recording software OBS.
